Wrestling involves inherent risks including bruises, sprains, fractures, and concussions. I voluntarily enroll my child knowing these risks exist and cannot be fully eliminated through supervision or instruction. I accept all such risks as a condition of participation*

I release Team Poeta Wrestling Camp, its owners, coaches, and staff from claims arising from injury or illness during camp activities. This release covers negligence but not gross negligence or intentional misconduct.*

I authorize camp staff to seek emergency medical care for my child if I cannot be reached. I accept financial responsibility for any costs not covered by my insurance. Staff will contact me before non-emergency decisions whenever possible.*

My child will treat all campers, coaches, and staff with respect. Bullying, hazing, harassment, or use of prohibited substances will result in immediate dismissal without refund. The camp director's decision in disciplinary matters is final.*

I grant permission for Team Poeta Wrestling Camp to photograph or video my child for promotional use including social media and the camp website. No full names will be published without additional consent.*
